Azerbaijan Mortgage Fund under the Central Bank of the Republic of Azerbaijan announces the following purchases

1. The request for proposals in connection with the motor lorry insurance of Azerbaijan Mortgage Fund under the Central Bank of the Republic of Azerbaijan.
2. The request for proposals in connection with the compulsory insurance of administrative building and civil liability associated with the use of the administrative building, as well as insurance of movable property of Azerbaijan Mortgage Fund under the Central Bank of the Republic
3. The request for proposals in connection with medical insurance of employees of Azerbaijan Mortgage Fund under the Central Bank of the Republic of Azerbaijan.

The value of participation in a procurement is 50 (fifty) £. The value of participation must be transfered to the following account of the Azerbaijan Mortgage Fund.

The applicant, who participate in procurement, must submit the following information about himself:

1. Application for participate in procurement;
2. Bank document confirming the payment of the participation fee;
3. Approved by a notary public copy of documents about applicant's legal status and work experience with notary procedure (charter, state registration certificate or extract from registry, license, Tax ID, bank details etc.);
4. A document confirming the authority of a person who is authorized to sign the proposal and procurement contract (on the order of appointment);
5. The bank statement of financial position of the last year of applicant's activities;
6. Approval of the financial statement of ativity during the last financial year by an independent auditor;
7. The reference about lack of taxes and other overdue compulsory payments in Republc of Azerbaijan(the Ministry of Republic of Azerbaijan and the State Social Protection Fund);

Collection of main conditions for procurement and a document confirming the payment of the participation fee (receipt) can be obtained from the coordinator with submitting.

The sealed and signed proposal envelopes on procurement will be taken until on April 09, 2014 at 06:00 pm, and all other required documents until on March 28, 2014 at 06:00 pm will be taken by the police in the administrative building located in Baku Azerbaijan Mortgage Fund, Bul-Bul Avenue 40. The envelopes received after the deadline will be returned unopened. Envelopes on proposition will be opened at 03:00 pm on April 10, 2014 in the administrative building of Azerbaijan Mortgage Fund. Applicants are invited to attend the opening of envelop by written notice to the authorized representative's name to procurement commission. In this case, the person must present a document certifying authority or power of attorney.
